From clinical to community care: exploring the system journeys of patients with serious mental illness

Abstract

This project endeavoured to explore the effectiveness of mental health and complex multidisciplinary teams (MDTs) and to gain insights into the system journeys of hard-to-reach patients with serious mental illness (SMI). We aimed to understand the environment and ecology of the Clinical to Community landscape, which informed rich visual maps of the patient experience of services – including statutory, voluntary, and other services or people that they may engage with.

This report was produced by Dr Meghánn Ward, Professor Tom Grimwood and Dr Laura Snell at Health and Society Knowledge Exchange (HASKE), University of Cumbria.
